At Mercy Community, we have a dedicated Disability Support stream which operates right across Queensland delivering services under the NDIS, QCSS and NIISQ funded programs. Disability Support is split into 2 main teams:

- 1) Individualised Support (which includes Supported Independent Living, In home, Community Participation and Supported Employment) and 2) Support Coordination. Each of these teams assist a broad range of people who may be living with physical, intellectual and psychosocial impairments.

Reporting to the General Manager of Disability Support, we are looking for 2 Practice Leads to assist each of these areas with staff support and development, expertise, guidance and leadership in best practice. This is a key role helping to increase our teams' capability and performance, as we strive to provide high quality service outcomes for the people we support.

**Examples of your deliverables**:

- Work collaboratively with operational management to identify service needs, deliver appropriate solutions and assessing effectiveness
- Translate research and practice evidence into resources and tools for use in service delivery.
- Document and maintain practice models used
- Facilitate case reviews, critical incident reviews and other data analysis to identify improvement opportunities to processes, systems and practice approaches.
- Provide secondary consultation and advice on current support scenarios and interventions.
- Incorporate person-centred, strength-based practice into service delivery models and associated operational tools.
- Design and facilitate ad-hoc learning opportunities as required to contribute to the ongoing professional development of team members.
- Contribute to the maintenance and evolution of role specific learning frameworks.
- Provide feedback on individual and team performance to manage risk and support employee development.

Along with your experience in developing and implementing processes and resources to build practice capability in others, you will also possess a:

- Tertiary degree in Social Work, Psychology, Human Services, or related discipline with a minimum of five (5) years' experience within the disability sector, health or associated human welfare sectors
- Positive and friendly personality, who is open, flexible and willing to be part of a broad team working together.
- Queensland Driver’s License;
- Current Yellow Card Exemption or NDIS Workers Screening Clearance
- NDIS Worker Certificate (this is free to do online)
- Current Blue Card
- Minimum of 2 COVID-19 vaccinations
