**About the Role**
We are seeking compassionate and reliable **Support Worker** to join our team. In this role, you will provide personalised support and resources to Participants, enabling them to continue living independently in their homes and within the community.
You will work under the guidance of medical professionals and supervisors, assisting with daily living, health, and wellbeing needs while ensuring services are delivered with care, respect, and in line with Community Support Pty Ltd’s policies and procedures.
**Key Responsibilities**
- Deliver individualised services in accordance with Participant support plans.
- Monitor Participant wellbeing and promptly report any concerns or changes to the Supervisor.
- Complete all required documentation accurately and on time.
- Assist with transportation, light domestic duties, personal hygiene, and meal preparation.
- Support health and wellness activities as required.
- Safeguard Participant and company property against loss, theft, or damage.
- Attend staff meetings, training sessions, and performance reviews as directed.
- Follow all Work Health and Safety requirements to ensure a safe workplace.
- Participate in continuous improvement initiatives.
- Uphold Community Support Pty Ltd’s Vision, Values, and Purpose.
- Comply with all policies and procedures outlined in the Staff Handbook.
- Undertake additional duties as requested by the Supervisor.
**Essential Criteria**
- Current and clear Criminal Record Check.
- Certificate III in Individual Support, Disability Care, or working towards qualification.
- Blue Card (as required under state legislation).
- Current local Driver’s Licence (international licences accepted while on a visa) and access to transport.
- Current First Aid and CPR certificate, or willing to get.
- Completion of NDIS Worker Orientation Module.
- Willingness to assist with daily living and personal care tasks.
- Reliability, productivity, and a positive attitude towards customers.
- Strong teamwork and relationship-building skills.
- Commitment to self-improvement and professional development.
- Good written and verbal communication skills in English.
**Desirable Criteria**
- Reliable motor vehicle with valid registration and comprehensive third-party insurance
- Mobile phone with internet access compatible with company requirements.
- Previous experience in community care, health/aged care, not-for-profit, or community services sector.
