**Your new role**:
As a Youth Case Manager, you’ll provide transitional support to young individuals and families aged 15-25 in Melbourne's Eastern suburbs, addressing homelessness or the risk of becoming homeless.

You’ll make an impact by providing engaging case management and outreach support to young people from a diverse range of backgrounds, assisting them in accessing relevant services and programs, and helping them break the cycle of homelessness.

As part of the Creating Connections network, you'll contribute to the Victorian Homeless Strategy, pioneering innovative solutions to prevent youth homelessness and streamline referral processes across the Eastern Metropolitan Region.

It is a full-time, max-term role covering parental leave until January 2026. We can also consider 0.8 or 0.9 FTE. Talk to us about what you are looking for, and we can see if we can make it work.

**Your skills and experience**:
You will also bring with you:

- A qualification in Community Services, Welfare, Social Work, or a related discipline, with a minimum Diploma level.
- Proven experience in the homelessness sector and knowledge of relevant referral pathways.
- Proven track record of working effectively with young people, particularly those with complex needs.
- Strong networking and liaison skills, with the ability to collaborate with schools and community organisations.
- Excellent skills in managing challenging clients and navigating complex issues with empathy and professionalism.
