**Join a growing Home Care / Support At Home Organization**

We are seeking a compassionate, organized, and client-focused Care Partner / Case Manager to join our dedicated home care team. This role is central to coordinating and delivering person-centred care to older adults and individuals living with chronic conditions or disabilities in the community. You will act as the primary liaison between clients, their families, healthcare providers, and support workers, ensuring all aspects of care are tailored, respectful, and empowering.

**Key Responsibilities**
**Client Assessment & Onboarding**
- Conduct comprehensive in-home assessments of clients’ physical, emotional, cognitive, and social needs.
- Collaborate with clients, families, and other stakeholders to develop tailored care plans that reflect goals, preferences, risks, and cultural values.
- Review medical history, diagnoses, and care needs to ensure appropriate support services are established.

**Care Coordination**
- Organize and manage a package of home care services including personal care, domestic support, social support, allied health, transport, and respite.
- Match clients with suitable support workers based on care needs, language/cultural compatibility, and preferences.
- Schedule and coordinate services in line with budget, care plan goals, and client routines.

**Ongoing Case Management**
- Maintain regular contact with clients and families to monitor wellbeing, adjust care plans, and ensure satisfaction.
- Conduct regular reviews of care plans (e.g. every 3-6 months or as needs change) and update documentation accordingly.
- Monitor service delivery, compliance, and quality of care provided by support staff and service partners.

**Advocacy and Client Empowerment**
- Support clients and families in navigating the aged care or disability support systems, including My Aged Care or NDIS (if applicable).
- Educate clients about their rights, choices, and the use of their care budget.
- Advocate for additional services, equipment, or medical support when needed.

**Documentation and Compliance**
- Maintain accurate case notes, assessments, risk management plans, and progress reports in the client management system.
- Ensure compliance with relevant legislative and funding body requirements (e.g. Aged Care Quality Standards, CHSP, HCP guidelines).
- Report incidents, complaints, and risks as per policy.

**Skills and Attributes**
- Empathy, emotional intelligence, and commitment to person-centred care.
- Strong communication and interpersonal skills, including conflict resolution.
- High level of organizational ability and time management.
- Cultural competence and sensitivity to diverse backgrounds.
- Ability to work autonomously while contributing to a multidisciplinary team.

**Qualifications and Experience**
- Certificate IV or Diploma in Community Services, Aged Care, Case Management, Social Work, Nursing, or related field.
- Experience working in home care, aged care, community services, or case management (1-3 years preferred).

Experience working as NDIS Support Coordinator
- Valid driver’s licence and access to a reliable vehicle.
- Police check and Working with Vulnerable People clearance (or equivalent) required.

**Work Conditions**
- Community-based role requiring home visits across a designated geographic area.
- May involve occasional after-hours check-ins or emergency response.
- Use of mobile device and care management platform (e.g. Turnpoint, Procura, iCare) for documentation and scheduling.

**Desirable**
- Understanding of Home Care Packages (HCP), Commonwealth Home Support Program (CHSP), or NDIS and Support at Home
- Multilingual abilities or experience working with CALD (Culturally and Linguistically Diverse) communities.
- Experience supporting clients with dementia, mobility issues, or complex health needs.

Pay: $70,000.00 - $80,000.00 per year
