**About the **Community Aged Care Services**

Community Aged Care Services offer a multidisciplinary service response within an integrated and comprehensive system of aged care at Austin Health, in the community, and as a contributor to State-wide Programs including:

- Heidelberg Aged Care Assessment Service (ACAS), which undertakes assessments for Australian Government subsidised services both in hospitals and in the client’s home.
- Austin Health Transition Care Program, offering case management, targeted therapy and implementation of services based on individually assessed needs to maximise function and independence.
- Austin Health Restorative Care Program, which provides short-term support and active management for people at the conclusion of a hospital episode who require more time and support in a non-hospital environment.

**About the role**

The Manager of Community Aged Care Services leads a team of highly skilled and passionate allied health and nursing clinicians and is responsible for the daily operations of the Community Aged Care Services. Responsibilities include (but are not limited to) development of operational plans, supporting and developing staff, ensuring compliance with legislative and funding requirements, participation in statewide meetings, and working with other members of the Continuing Care leadership team to ensure high quality, coordinated patient care.

- Experience in leading teams, both clinical and operational
- Knowledge and experience in aged care and community-based care
- Ability to support staff and lead by example
- Ability to develop, negotiate and manage agreements, contracts and commercial arrangements
- Customer and quality focused approach to service delivery
