**About the Role**

We are seeking a **Family Support Worker** to join our dedicated team supporting NDIS participants and their families. This role is ideal for someone passionate about making a positive impact in the lives of people with disabilities and their carers. You will provide practical and emotional support, assist in navigating NDIS plans, and advocate for families to ensure they receive the necessary services and resources.

**Key Responsibilities**

**Family and Participant Support**
- Provide hands-on, practical support to NDIS participants with disabilities, assisting with daily living activities such as personal care, mobility, meal preparation, and community participation.
- Support families in navigating NDIS plans, funding options, and service coordination.
- Assist in developing strategies for managing caregiving tasks, behavioral challenges, and daily routines.

**Liaising and Stakeholder Collaboration**
- Act as a key liaison between internal teams (e.g., Support Coordinators, Allied Health Professionals) and external stakeholders (e.g., therapists, schools, community organizations).
- Build and maintain strong relationships with families, service providers, and the wider community to enhance support networks.

**Program Development and Implementation**
- Design and deliver support programs tailored to the needs of families and participants, aligned with NDIS goals.
- Organize workshops and peer support groups for families and carers.

**Case Management and Coordination**
- Develop, implement, and review family support plans based on individual assessments and participant goals.
- Ensure seamless service delivery across multiple providers.

**Monitoring, Reporting, and Compliance**
- Maintain accurate and confidential records, including case notes, progress reports, and incident reports.
- Prepare reports for management and NDIS bodies to track participant progress and service effectiveness.

**Advocacy and Resource Access**
- Advocate for families and participants to ensure they receive the appropriate support, funding, and services.
- Assist families with NDIS plan reviews and submissions for additional support.

**Leadership and Supervision**
- Provide mentoring and guidance to Disability Support Workers to ensure high-quality service delivery.

**Crisis Management and Problem-Solving**
- Respond to emergencies, behavioral incidents, or crisis situations involving participants or families.

**Continuous Improvement and Professional Development**
- Stay updated on NDIS policies, regulations, and best practices.
- Participate in professional development and training sessions.

**Required Qualifications & Skills**
- **Diploma or Degree** in Social Work, Community Services, Disability Services, or a related field.
- Experience working with individuals with disabilities and their families, preferably within the **NDIS system**.
- Strong understanding of **NDIS plans, processes, and service delivery** requirements.
- **Excellent communication, advocacy, and interpersonal skills**.
- Ability to **manage sensitive and confidential information professionally**.
- Knowledge of **community resources and disability support services**.

Pay: $80,000.00 - $90,000.00 per year
