**Key Duties**
- Provide support that ensures the client is center of their decision making and enable them to achieve the desired outcomes of their plans, goals and aspirations in partnership with their families and carers.
- Manual Handling, assist clients with mobility and transferring, using appropriate lifting techniques.
- Help clients with daily personal hygiene, including bathing, dressing, grooming and toileting.
- Monitor and report changes in clients’ health and wellbeing to relevant health professionals and management
- Administer medication as prescribed and keep accurate records
- Assist clients with meal preparation and feeding
- Assist with household task such as cleaning, laundry and shopping
- Accompany clients to appointments, social activities, and community events
- Facilitate clients’ engagement in hobbies, interests, and community programs
- Implement individual support plans and behavior management strategies.
- Maintain accurate and up to date records of clients’ activities, progress and any incidents
- Complete daily reports, shift notes and communicate effectively with teams’ members and management
- Advocate for clients’ rights and needs within the community and service environment
- Ensure clients are always treated with dignity and respect
- Assist clients in developing life skills and independence, including money management, cooking, and personal organisation
- Encourage and support clients in setting and achieving personal goals.
- Develop appropriate, supportive working relationships with customers and their families/carers.
- Ensure that supports are provided in accordance with the National Disability Services Standards and other relevant legislation and are active in advocating the rights of people with a disability.
- Work collaboratively with other support workers, health professionals, and family members
- Participate in team meetings, training sessions, and professional development opportunities

**Compliance Requirements**
- NDIS Worker Screening Check
- Working with Children Check
- Provide First Aid (HLTAID011)
- Provide CPR (HLTAID009)
- Valid Australian Drivers Licence
- Manual Handing - certificate or training
- Medication Administration - certificate or training
- Evidence of Covid19 Vaccination Status
- NDIS Worker Orientation Module “Quality, Safety and You”

**Desirable**
- Certificate III in Individual Support or above
- Palliative Care
- Peer Support
- Certificate in Mental Health
- CJP/Forensics
