**Clozapine Sahred Care Co-ordinator - Senior Clinician**

**Outer Community Mental Health Team - MWAMHS**
- Part-Time position based in Melton
- Coordinate and oversee the Clozapine program within a busy and dynamic community mental health team.
- Work collaboratively with NWMH services, external agencies, service providers and other health professionals to ensure continuity of care for consumers and their family/carers around clozapine treatment and care planning.

- As the senior clinician you will help in the treatment of consumers conscriped Clozapine as well as contribute to organisation-wide and service/ division initiatives and planning activities

**Your new role**
- Demonstrate your extensive experience working as a mental health nurse within a multidisciplinary care team in a community setting
- Understand the clinical risks associated with clozapine therapy
- Work within the associated protocols and monitoring requirements for all registered consumers prescribed clozapine with the service
- Work collaboratively with primary health care providers e.g. GP practices
- Pro-actively contribute to innovative service planning and development
- Lead with kindness, role modelling open communication and a desire to support and enhance clinical care across the team

**Your Contribution**
- Extensive experience in assessment/treatment of people with mental illness / psychiatric disability
- Demonstrated ability to provide leadership within a multidisciplinary team
- Highly developed interpersonal and communication skills (written and verbal)
- High level awareness of legislation governing mental health practice
- Strong commitment to consumer-focused service delivery
