**The Role**:
Due to growth, we are looking for two Home Care Service Advisor's reporting to the Home Care Regional Manager to support the coordination and management of care to our clients who are on Home Care Packages in the Blacktown and Southwest Region.

**Full-Time role based at Merrylands, Flexible days and hours, Monday-Friday with no weekends or on call.**

As a Home Care Service Advisor you will:

- Complete holistic assessments
- Develop appropriate care plans
- Manage client’s care needs within Home Care Package funding
- Manage referrals to other services when required
- Manage and provide service delivery to a portfolio of home care clients and their families.

To be successful in this role, you will have:

- Qualified Enrolled Nurse or Registered Nurse
- Cert 4 in Aged Care
- Excellent communication and interpersonal skills
- High level of organisational and time management skills
- Intermediate-Advance Microsoft Office skills
- Experience in home care, with a qualification in Community Services, Aged Care, Community or Case Management (highly desirable)
- Driver's license and car with comprehensive insurance (required)
- Evidence of COVID-19 vaccinations
- Evidence of flu vaccinations
