**Support Coordinator - About the Opportunity**

An exciting opportunity exists for a highly motivated and experienced person, passionate about
supporting our participants, their family, and carers in identifying and connecting with appropriate
support providers to overcome barriers, develop skills and achieve the objectives of individual NDIS
Plans. The Support Coordinator plays an integral role for Inala participants in interpreting their NDIS
plan, assisting them with establishing the supports in their plan, developing their capacity over time
to coordinate and manage their own supports, and addressing challenges and obstacles which they
may encounter.

Reporting to the Disability Services Manager, this is a full-time role based at Cherrybrook in Sydney’s
northwest, with working hours Monday to Friday, office hours.
- In your role you will be responsible for:
- Empowering NDIS participants and their families to understand, implement and review
- their NDIS plans- Providing advice to participants and their carers on the evolving environment of the
- NDIS and services available- Actively assisting participants in successfully coordinating their support needs and in
- achieving objectives of their NDIS plan- Researching and identifying providers and other relevant stakeholders and building
- strong working relationships- Preparing reports, monitoring expenditure, and completing all associated administrative
- work- Assisting participants with resolving crises when they arise, and taking steps to mitigate
- potential risks- Arranging for any assessments required to determine the nature and type of funding
- required- Being the key contact for service issues, complaints, major changes, and plan reviews
- Assisting NDIS participants with preparing for their NDIS plan reviews by
- Assessing whether their goals were achieved and whether support funding is
being appropriately and efficiently used
- Identifying solutions to problems experienced in implementing the plan, and
considering new goals
- Assisting participants in deciding what actions to take to achieve goals in
relation to exploring housing options and life transition planning

Support Coordinator

**Essential criteria**
- Experience working in Support Coordination, or a similar role, identifying and linking

participants to services that cover a diverse range of support needs.
- Excellent interpersonal skills that value individual and cultural diversity and which foster

productive working relationships with a wide range of stakeholders.
- Demonstrated ability to identify and provide a range of solutions to complex challenges,

barriers and risks that face participants and work with stakeholders to manage such
challenges.
- Demonstrated experience working collaboratively with a small team where you show

excellent communication skills and a willingness for continuous learning.
- In-depth working knowledge of the National Disability Insurance Scheme (NDIS), rules and

operational guidelines and experience in coordinating a range of community-based supports
and methods to achieve participant goals.
- A ‘Can Do’ Attitude with a Customer Service Focus
- Read and understand the Inala Vision, Core Values and Principles of the Inala Ethos and have
- a preparedness to work within the Ethos of Inala- Hold and continue to hold NDIS Worker Clearance and an entitlement to work in Australia***
