**About Us**

Nepean Blue Mountains Local Health District (NBMLHD) is a wonderful place to expand your career and grow your skills and knowledge. As a recognised leader in the healthcare industry, we provide a range of public health services to the Nepean, Blue Mountains, and Lithgow Region.

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

The Pregnancy Family Meetings (PFM) program utilises family meetings with an independent facilitator to promote early engagement and interagency planning with pregnant women and families at risk of their newborns entering Out of Home Care at birth. Early engagement and partnerships between Health and the Department of Communities and Justice aims to reduce risks and the number of infants assumed into care.

The program utilises a strengths-based model which promotes a participatory and transparent process for parents and families.

The Facilitator is responsible to be the primary Facilitator of the Pregnancy Family Meetings across NBMLHD. This includes undertaking clinical work across Nepean Blue Mountains Local Health District (NBMLHD) and Department of Communities and Justice (DCJ) (WS&NBM District). Including to:

- Work directly with the family and agencies to schedule and then facilitate Pregnancy Family Meetings.
- Develop and maintain a group of appropriately skilled PFM Facilitators from within NBMLHD
- Develop and foster strong interagency partnerships to sustain and further develop the program

**What you will bring to the role**
- Social Worker - Degree in Social Work from an accredited course providing eligibility for membership of the AASW. Counsellor - Minimum of a bachelor’s degree in counselling and eligibility for membership of the ACA or Psychotherapy and Counselling Federation of Australia or have completed the Djurrawang program in NSW Health. Welfare Officer - Hold a minimum of a bachelor’s degree in a professionally accredited course recognised by the ACWA. Snr Psychologist - Psychologist registered with AHPRA and have completed a minimum of one-year full time equivalent at the 9th year of service for a psychologist. Clinical or Forensic Psychologist - Accredited post-graduate master’s or doctorate degree in clinical or forensic psychology and general registration as a psychologist with AHPRA and endorsement in the area of clinical or forensic psychology or working towards endorsement.
- Demonstrated skills in facilitating case conferences, case meetings, group supervision or similar meeting/process. Preferably with the involvement of family members and other key stakeholders
- Demonstrated high level of understanding of the needs of vulnerable people including people who have experience childhood violence, abuse and/or neglect, or domestic violence. Understanding how this can impact on a person’s capacity to parent
- Demonstrated understanding of the physical, emotional, and psychological needs of babies and children, and the role of the parent in supporting the development of the child
- Thorough knowledge and demonstrated high level skills in child protection practice, and knowledge of Child Protection Legislation, Policies and Procedures
- Demonstrated high level interpersonal, written, and verbal communication skills including interagency negotiation skills, ability to provide timely and relevant advice, analytical skills, and problem-solving skills
