**Social Worker / Care Coordinator**

**Rehabilitation Unit**

**We are looking for a dynamic and innovative Social Worker or Registered Nurse with experience in Case Management including discharge planning.**

The Rehabilitation Unit is a 45 bed unit that supports patients in recovery from acute or chronic illness or surgery prior to returning home.

**About the role**

The position is responsible for:

- Assessment and active management of patients with complex discharge needs or at a risk of an extended stay
- Participation in multi-disciplinary Case Conference
- Formulation of discharge action plans and referral to community services, transitional care and My Aged Care
- Guidance through the Residential Placement process for patients and carers

The role works within the Case Management team and is covered 4 days a week.

**Skills and Experience**

Essential:

- Degree in Social Work and an accredited member of AASW, or Nursing Registration with AHPRA
- Demonstrated ability to efficiently manage case load demands
- Experience in aged care, residential placement process and discharge planning
- Highly developed interpersonal skills and ability to communicate with a wide range of customers
- Demonstrated ability to provide effective customer focused services
- Commitment to professional development and maintenance of current evidence based practice
- Flexible positive approach to work
- Demonstrated proactive approach to problem solving and conflict resolution

Desirable:

- Experience working in a private hospital setting
