**The Position**

**Position Title**: Lifestyle Coordinator
**Classification**: Lifestyle Coordinator Grade 3-IN33
**Salary per hour (exc. super)**: $31.98
**Hours per fortnight**: 64 hrs
**Employment Status**: Permanent Part time
**Other requirements**: COVID-19 Vaccination, Influenza Vaccination, Police Check, NDIS Worker Screening Check, Drivers Licence (manual), Food Handlers Certification

**Role Description**

The Lifestyle Coordinator liaises with the consumer, representatives, Nurse Unit Manager and care team to develop, plan, implement and evaluate a range of activities in response to the consumer's preferences, choices and goals. The Lifestyle Coordinator is responsible, as part of the team, to contribute to the continuous improvement of Lifestyle programmes and activities.
