**What We're Looking For**
**You will possess**:

- Relevant experience and/or transferrable skills
- Familiarity with Psychosocial Disability (advantage)
- Basic understanding of the NDIS and Support Coordination framework
- Ability to build networks and collaborate with both internal and external stakeholders
- Thrive in a dynamic environment with a commitment to continuous improvement practices

**About the Role**
This is a full-time position based in Surrey Hills, Victoria. Where you will be joining an experienced team with a genuine dedication to supporting clients through NDIS. Your role will involve working closely with clients to effectively utilise their NDIS-funded supports, connect them with appropriate service providers and allied health professionals, and monitor their progress towards achieving their plan objectives.

**Responsibilities**:

- Collaborate with clients and families to help them achieve their NDIS plan goals
- Empower clients with a deeper understanding of their plans and how to implement them
- Develop robust plan implementation strategies for the NDIS plan duration
- Cultivate and maintain strong relationships with service providers and allied health professionals
- Deliver comprehensive plan review reports to NDIA, clearly communicating ongoing client needs
- Offer effective, transparent, and responsible services to clients
