**About the Role**:

- This role of the support worker is to provide exceptional service whilst supporting clients to meet their goals as set out in their NDIS plan._
- This includes;_
- Seeking young energetic support worker to work with a 15 yr old participant with ASD-II, knowledge re gaming and keen on assisting him with outdoor activities. Seeking support for school pick up and afternoon supports in the community and catching up with appointments._

Working autonomously
- with casual catchups with team leader to provide appropriate levels of support to maintain and improve the client’s level of independence. This may include community participation, skill building, relationship building, and various other needs.
Respond to the client’s needs and collaborate positively with their families, support people and others in their lives.
Keep accurate and complete records of activities via progress notes
Work within the NDIS Code of Conduct.

**Who you are**:

- To be successful in this role we are looking for someone who has Certificate III in Individual Support (or equivalent), and the following attributes;_
- Working with Children Blue card
- Completion of NDIS Worker Orientation Module
- An understanding of NDIS and a commitment to its principles
- Ability to work in partnership with NDIS participants to achieve their goals
- A passion for assisting individuals with a disability to improve their quality of life and to reach their potential
- Driver's Licence and own car, and a willingness to use for transporting clients (where required)
- COVID-19 Vaccination, it is a Queensland Government requirement that all individuals working within Healthcare Settings, including Disability Support Services are vaccinated against COVID-19, or evidence of medical exemption.

**Due to the support needs of our clients, you must be available and willing to work a varied pattern of shifts across a 24/7 roster (including short notice, short shifts, night shifts, weekends and public holidays) to be considered for these roles.**

Pay: $31.75 - $45.75 per hour

Expected hours: 6 - 15 per week
