**About the Program**

Join one of our newest programs in one of our fasted growing service portfolios Support services for people with mental health concerns in Residential Aged Care Homes (RACHs) aims to enhance the quality of life for older people through improved access to primary mental health care within a person-centred stepped care approach.

This program will deliver in-reach health care services to residents in aged care homes. Our services will target residents with a diagnosed mental illness or those who are assessed as at risk of developing mental ill-health. This is a hugely rewarding role where you can make a visible, lasting impact and improve someone’s quality of life. The program is led by a compassionate, curious, and non-judgemental leader, passionate about supporting staff and asking, ‘why not, and what’s next?’

**Your opportunity**

As a Residential Aged Care Group Facilitator, you will complete a brief intake and assessment process to determine the residents’ suitability to participate in our groups and/or recommend referral for those with higher support needs. Our team works out of two offices - one located in Toowoomba and the other in Ipswich. In this role, you will be based in our Ipswich office. We are a small team, so it's important that you feel comfortable working independently.

This is an autonomous role that involves regular travel within the region. You will travel to several aged care home and regularly run groups of 2 - 3 hours duration. The groups will cover topics such as understanding mental health, reframing strategies, mindfulness and enhancing social connectedness. The groups are grounded in acceptance and commitment therapy as a brief intervention, to support residents in managing mental wellbeing independently and improve social, emotional and psychological health.

**Your responsibilities will also include, but are not limited to**:

- Confidently facilitate training and group information sessions to participants.
- Engage with local providers, developing positive relationships, and building a local presence.
- Collect and administer outcome measures and provide feedback for continuous program improvement.
- Build rapport with participants, linking them to appropriate information and referral options where appropriate.Be the face of LLW and the program when visiting residential aged care homes.

You will have the support of an experienced Team Leader, and Allied Health Practitioner - Social Worker in assessing referrals and receive further support.

This position would suit an individual with an aged care, nursing or nursing assistant background and/or those with mental health support work experience. This role will not provide ongoing counselling to participants, so you don’t need a strong background in providing this type of support.

**To succeed in this role, there are some essentials you’ll need**:

- Understanding of cognitive decline in the ageing population, and the effect on overall well-being.
- Ability to organize and deliver recreational activities and experiences to older persons alongside the ability to be creative and take initiative.
- Excellent stakeholder management skills to enable you to build effective working relationships with our key stakeholders.
- Proven effective engagement and communication skills with older persons.
- Demonstrated ability to solve complex problems.
- Ability to work collaboratively, cooperatively, and effectively as part of a multi-disciplinary team.
- Possess the essential qualities of compassion, empathy, active listening skills and maintain a strong work ethic.
- Demonstrated understanding of mental health conditions and the mental health sector, especially in regard to older persons mental health.
- Hold an Australian Driver’s LicenceTertiary qualification (or working toward) in Social Work, Psychology, Social Science, Health, or a related field is highly regarded.
