**About Us**

Nepean Blue Mountains Local Health District (NBMLHD) is a wonderful place to expand your career and grow your skills and knowledge. As a recognised leader in the healthcare industry, we provide a range of public health services to the Nepean, Blue Mountains, and Lithgow Region.

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

A Permanent Full time opportunity is now available for a Social Worker to join the PCCH Transitional Aged Care Teeam, located at Springwood Community Health Centre.

This community role will see you provide Social Work services to clients of the TACP, C&CC and OPERA Programs, in accordance with professional codes of ethics, the NSW Health Code of Conduct and NBMLHD/PC&CH Policies and Procedures. You will manage a clinical caseload, providing a professional level of service which is commensurate with their years of experience. Level 1 staff are newly qualified employees who will work under discipline specific professional supervision.

The Social Worker may also provide a service to the NBMLHD Chronic and Complex Care Social Work Team and the OPERA Team, as required.

**What you will bring to the role**
- Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers and a commitment to maintaining a professional skill and knowledge base. Overseas qualifications must supply the AASW Overseas Assessment number.
- Current NSW State Driver Licence, including P1 or above and willingness to use for work purposes. This may include driving long distances across NBMLHD.
- Clinical skills in assessment, planning and evaluation of social work interventions with adult clients who have chronic and complex health conditions and the capacity to work with this client group in a community setting. (Level 1 staff are newly qualified employees with less than three years clinical experience, who will work under discipline specific professional supervision. Level 2 staff have at least three years clinical experience, and are expected to be competent independent practitioners, with who can work under mínimal direct supervision).
- Understanding of the psychosocial impact and management of chronic health conditions, ageing and disability and knowledge of grief and bereavement theory, commensurate with years of experience.
- Commitment to professional development and training and willingness to engage positively in clinical supervision and performance development processes.
- Knowledge of and commitment to evidence based practice and the ability to demonstrate how evidence based practice has been incorporated into your professional work.
- Ability to work independently at a professional level as well as the ability to work productively as a member of a multidisciplinary team in a community health setting.
