Bare Care are seeking a Team Leader within our 24 hour Supported Independent Living services Across Sydney.

**Making it happen - your Role. **We have an opportunity for a Team Leader to join us on a full-time basis, leading a team, supporting our Properties.

We are all about working to co-design the support delivery with participants and their family that enables maximum choice and control for the people we support.

This is a fantastic opportunity to further develop Bare Care’s reputation as an outstanding provider in the area. To try new things and be more flexible in how we deliver a new era of support. Leading good practice and developing excellence in service, you will provide management support whilst supporting people to build community relationships.You will maintain and enhance our high standards and build strong working relationships with our customers, regulators and other important stakeholders.

This will include taking direct management responsibility for referrals, health and safety, compliance issues and financial control. You will also play an important strategic role in defining, developing and implementing our growth plans.

**To be a great Team Leader, you will need**:a relevant qualification within disability (Cert IV or higher)
demonstrated experience in a similar role overseeing/coordinating disability accommodation services and managing a team of staff in a residential setting, including supervision and mentoring.
Experience working with residents with complex needs and behaviours of concern,
Ability to engage, build and maintain solid stakeholder relationships coupled with your sound negotiation and dispute resolution skills
Experience writing and coordinating incident reports
A current drivers license, mandatory.

**Main Duties **Proactively promote a home like environment that enhance the well-being of people with disabilities and is welcoming of families, friends, and other visitors
Confidently provide mentoring and leadership to a team to provide flexible services which meet the individual needs of people we support.
Implement partnership in care approach to enable optimal health and well-being to be achieved, including the active involvement, of people with disabilities, their families and referrers in the identification of health care needs and the training of staff around individual needs and preferences.
Develop, review, and actively maintain all the resident care/support plans in conjunction with all stakeholders, residents, families, and other service providers to ensure health care needs of residents are achieved.
Ensure that restrictive practice approvals and reporting is in place, inclusive of compliant behavior support plans.
Ensure a person centered and self-directed approach is embedded within the culture of the facility to maximize independence.
Onboarding needs for Staff and Participants
Participation in an on-call roster which includes staff and resident support outside normal business hours, weekends and overnight.
You must have a positive attitude towards issues of intellectual disability and mental health and a comprehensive understanding of these issues in practice. With the ability to act as lead practitioner and develop the team’s skills via supervision and on the job coaching, you will have the aptitude to respond to the needs of a 24 hour service to include on call arrangements and a need to provide management cover when required and therefore a flexible approach is essential.
