**Hours**:This position will be Part-Time, 0.4 FTE.
**Location**:Port Adelaide, South Australia
**About the Role**:
The Clinical Care Planner will work as part of a multidisciplinary mental health team to provide clinical care coordination to assist people with complex mental health concerns
The Clinical Care Planner will be responsible for:
- Working alongside patients, their GPs and other health professionals involved in their care to develop a joint care plan aimed to improve the ongoing management, coordination and treatment of complex and severe mental health management in general practice.
- Enhancing the effectiveness of a stepped-care approach to mental health care in primary care settings
- Liaison and consultation with other professional employees
**Experience Requirements**:
- Demonstrated experience working within a fast-paced environment.
- A minimum 2 years supervised experience in the area of mental health.
- A minimum 2 years relevant professional development and or supervision.
- Experience in delivery cognitive behaviour therapy and other evidence based interventions.
**Qualification and Other Requirements**:
- Drivers Licence
- Current or willing to obtain a Working with Children's Check (WWCC)
- Knowledge and experience in using the Microsoft suite of software and databases.
- Qualified in the following disciplines and membership to associated professional body; E.g.: Psychology, Social Work, Occupational Therapy, Mental Health Nursing, Registered Nurse or Health care professional
- Working towards or current and full registration with the appropriate registration authority
