**Location**:Sonder Edinburgh North, Port Adelaide and other offsite locations as required

**About the Role**:
Join our multidisciplinary mental health team as a **Clinical Care Planner**, supporting individuals with complex mental health needs. In this role, you’ll coordinate care by working closely with patients, GPs, and other health professionals to develop tailored care plans. You'll play a key role in improving treatment outcomes through a stepped-care approach and providing professional consultation and liaison across services.

**Experience Requirements**:

- Demonstrated experience working within a fast-paced environment.
- A minimum 2 years supervised experience in the area of mental health.
- A minimum 2 years relevant professional development and or supervision. Experience in delivery cognitive behaviour therapy and other evidence based interventions.

**Qualification and Other Requirements**:

- Qualified in the following disciplines and membership to associated professional body; E.g.: Psychology, Social Work, Occupational Therapy, Mental Health Nursing or Health care professional
- Working towards or current and full registration with the appropriate registration authority
- Drivers Licence
- Current or willing to obtain a Working with Children's Check (WWCC)
- Current or willing to obtain National Police Clearance
