**About the Opportunity**

The Team Leader, Support at Home works closely with and reports to the Manager Community Connections to achieve person centered care practices for clients. The role leads and supports a large team of support at home coordinators, and ensures excellence in care that is responsive to the clients' assessed needs.

**Responsibilities include but are not limited to**:

- Manage the day to day delivery of high-quality care services by providing appropriate resources, leadership, direction, support and coaching to all staff;
- Establish and maintain standards of quality care through risk management, quality assurance and compliance activities;
- Ensure that human, material and financial resources are managed appropriately and within budget;
- The development, planning, implementation and evaluation of education and staff development programs;
- To monitor the quality of comprehensive contemporary assessment and care planning practices, ensuring the clients quality of life and independence is optimized.
- Demonstrated management experience, preferably in a not-for-profit aged care organisation
- Sound understanding of the regulatory requirements of all matters relevant to aged care accreditation
- Experience managing and leading a team in a home support setting, particularly through periods of change in culture, services, and approaches to Support at Home
- Demonstrated commitment and ability to meet client’s needs and manage expectations

You must also be willing to obtain a National Police Certificate before commencing employment as well as a functional assessment.
