Colbrow Care provide in home supports to a broad range of individuals including those receiving Aged Care at home - Private, HCP as well as NDIS, TAC & Worksafe clients.

While no two days and no two clients will be the same, our mission for ALL remains the same; to deliver excellence in personalised care solutions and services, allowing clients the option to live in the comfort of their own homes as independently as possible for as long as possible.

You will be responsible for the delivery of personalised support that enhances your clients' independence, social and community participation and helps them achieve their goals in accordance with their care plans.

You will support each client to maintain their independence and dignity by providing assistance with activities of daily living such as:

- _ Personal care_
- _ Social Support_
- _ Transport_
- _ Respite Care_
- _ Medication Monitoring_
- _ Domestic Assistance_
- _

To be successful in this role, you will have **AND/OR** be willing to obtain the following:

- _ Certificate IV in Aged/ Community Care, Disability or Individual Support_
- _ Proven experience in the field in this or a similar role/field_
- _ Current Police Clearance Certificate_
- _ Current Working with Children's Check_
- _ NDIS Worker Screening Check_
- _ First Aid Certificate (HLTAID011) and CPR (HLTAID009)_
- _ Mandatory competencies (as required by Colbrow Care)_
- _ Valid working rights in Australia_

You may not have extensive experience, but with a growth mindset and commitment, we will help you establish and grow your career as a Community Care Worker.
