We are looking for a Community Aged Care Coordinator with expierience to join our Team in Adelaide.

Role Purpose:

- Responsible for individualised care, support services for consumers including the coordination, scheduling and implementation in accordance with approved funding, guidelines, legislative requirements and company work instructions.
- Manage care and support which enables the consumer to live safely and independently at home while maintaining wellness, safety and connectedness, in line with consumers co-designed Care Plan and within the scope of practice of the role.
- Deliver positive outcomes for consumers, supporting them to reach their goals.
- Oversee individual service agreements, manage and monitor relevant budgets outcomes.

Qualifications:

- Minimum qualification of diploma in Human Services / Health / Community / Disability or equivalent.
- Current National Police Clearance - Aged Care (essential)
- Current First Aid Certificate (desirable)
- Current SA Drivers Licence (essential)
- Reliable vehicle & comprehensive insurance (essential)

**Job Types**: Full-time, Part-time, Casual
Part-time hours: 20 - 30 per week

Schedule:

- Day shift

Supplemental pay types:

- Overtime pay
- Penalty rates
- Signing bonus

Ability to commute/relocate:

- Prospect, SA 5082: Reliably commute or planning to relocate before starting work (required)
