**Monash Watch** aims to keep some of our most vulnerable clients, especially those with complex chronic health issues, healthier at home thereby reducing avoidable hospitalisations and Emergency Department presentations.

Monash Watch regularly calls patients to conduct a self-rated health check, and when there is a change to a patient’s responses the Health Coach is alerted to investigate and intervene.

**About You**
- You are experienced, flexible and dynamic Nurse that is looking for a change in career.
- We are a high performing team and we ensure that our client’s journey through Community Health is seamless and timely

You Are Also:

- committed to delivering patient centred care & a patient advocacy
- enthusiastic, compassionate & able to work collaboratively

What You Need:

- Demonstrated recent experience as a Registered Nurse
- Current AHPRA registration
- Valid Drivers licence
- Australian working rights
