- Melton Community Health Service - Outer Community Team is a multidisciplinary team comprising of Nurses, Social Workers, Occupational Therapists, Psychologists and Medical staff.
- We work collaboratively to support the mental health needs of individuals with mental illness who reside in the City of Melton and parts of Brimbank

**Your new role**:

- Assess and provide recovery focused treatment interventions to consumers with various psychiatric illnesses
- Coordinate medical, psychosocial and legal aspects of care delivery
- Engage with consumers, their family members and other service providers
- Develop and deliver evidence-based practice and appropriate treatment options
- Work in collaboration with your multi-disciplinary mental health team

**About You**:

- A motivated mental health clinician committed to continued learning and developing of your clinical skills
- Skilled in engaging consumers and family members, developing rapport and following up with community linkages / referrals to specialist services.
- Ability to work on a dynamic team and draw on the experience of your colleagues as well as being confident to work autonomously
- Excellent communication skills, both in verbal and written form.
