Full time ongoing position within the TAC Support Facilitation Team.
Case management, referrals, linkages and assertive outreach
SCHADS Level 4 - base salary $77,584.00 - $83,479.76 plus super & salary packaging

About You
You are motivated to improve the lives of those living severe mental illness. Are you able to effectively support people to achieve their desired outcomes. Are you reliable and consistent in your practice. Additional attributes we are looking for include:
Previous experience in Mental Health (professional, volunteer, lived experience) supporting people with complex needs; ABI / AOD / Mental Illness / Physical disability / dual diagnosis
Have a desire to learn more
Have an understanding of technical terminology in a clinical setting
Able to effectively and quickly build rapport with others
Are flexible, patient, display initiative and emotional resilience
Effective listening skills and an adaptabe communication style
Qualification/s in Mental Health, Social Science or other relevant qualification (or be actively studying) is beneficial

It is a requirement of this position to acquire and maintain a positive Working With Children’s Check, NDIS Worker Screening Check and a current & valid driver’s license.

About the Role

We assist & support individuals (consumers) who have a mental health condition and /or Acquired Brain Injury and/or disability as a result of a traffic or workplace accident. Our aim is to develop skills and abilities to handle day to day tasks. Our goal is to enable rather than do, to promote independence, development and confidence of the individuals. Our success will enable them to have the voice and choice to thrive in their local communities.

While ermha365 continues to operate face-to-face services through COVID-19 we are taking appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all COVID-19 standards, requirements and processes including (but not limited to) wearing appropriate PPE, screening and regular hygiene and cleanliness practices.

**The role entails**:
Community outreach
Case management
Communication with a wide range of health professionals
Working in a range of environments; community outreach, from home and using a telehealth model
Follow an established behavioural support plan
Developing professional relationships with individuals, their families and others in the community
Providing accurate and timely case notes and reports
Use of a variety of systems and navigate multiple platforms
Mon - Fri 9am - 4:36pm

In the short-term, this role will operate remotely. We have in place remote training and ongoing supervision. You will be connected to your new team via video conference/virtual meet ups that will offer peer support and guidance. You will be required to have use of a personal vehicle for work related travel and be reimbursed for travel expenses as appropriate.
