**Description**:
About the role
The NDIS ACS Coordinator’s key role function is to coordinate access, connection and support services for NDIS clients seeking assistance from ATSICHS. The role will assist in building connections and engaging support services while building capacity and enabling participants to live more independently and be included in their communities.
The NDIS ACS Coordinator provides support to staff, more specifically health practitioners and the community, to educate, implement and streamline processes to ensure eligible ATSCHIS clients are provided the support they required through the NDIS. The role works to skill ATSICS staff in understanding how the NDIS can assist their clients and assists in building knowledge and understanding of the scheme within the community.
Support fellow staff in a team environment to build collegiality and a sense of belongingness within the team and ATSICHS family. Staff will actively participate in team activities and contribute to a flourishing workplace culture that promotes the ethos and values of ATSICHS Brisbane as a long standing Aboriginal Community Controlled Organisation.
Duties
Effective case management and accurate data tracking/reporting
Working with Participants to achieve their goals in line with their NDIS plan
Develop, implement, monitor and review ATSICHS NDIS Service Level Agreements (SLAs) and where applicable, ensure participants receive timely and clear communication for SLA’s and quotes for participants
Create linkages with the community through communication and marketing and community engagement

Base salary of $68,151 and is negotiable depending on qualifications and experience

**Skills and Experiences**:
COVID-19 Requirements
- Immunization history showing you have received two doses of a Covid 19 vaccine.
**Essential**:
Certificate III or higher qualifications in Health Care, Social Assistance, Community Services or Administration.
Knowledge, understanding and/or awareness of the NDIS space and the challenges the participants may face
Demonstrated ability to record accurate and consistent case notes.
Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.
Strong organisational skills and ability to work independently and/or as part of a team.
Previous experience in working with culturally diverse people.
Ability to communicate sensitivity and effectively with Aboriginal and Torres Strait Islander people.
Sound knowledge of Microsoft Suite Applications.
Current Drivers Licence (Qld).
Current National Police Clearance or ability to obtain.
Working with Children’s Card (Blue Card) or be eligible to apply.
Desirable
Previous experience of working with Aboriginal & Torres Strait Islander clients.
Previous experience in working with the National Disability Insurance Scheme (NDIS).
