We are fully registered under the NDIS promoting choice and control, enhancing the quality of a person's life and advocating a person's rights in line with the person-centered philosophy.

We are currently seeking an experienced NDIS Support Coordinator that has experience working alongside people with a psycho-social disability. This is a Fulltime position but can also be Parttime.

Key Responsibilities include:

- Provide support to clients based on their NDIS plan, implementing all supports within the clients plan including formal, mainstream and funded supports.
- Build effective relationships with clients, families and stakeholders while maintaining professional boundaries.
- Ensure regular contact is maintained and recorded
- Supporting clients through NDIS plan reviews
- Assist clients to understand their funded supports and how to utilise their NDIS plan
- Minimum 2 years experience as a NDIS Support Coordinator
- A comprehensive understanding of mental health issues and the challenges individuals face
- Be passionate about pursuing a people focused career which genuinely impacts the lives of others.
- A thorough understanding of recovery and rehabilitation using a strength based and solutions focused practice
- Understanding of person-centred support
- Demonstrated experience working with individuals who live with a mental illness.
- Ability to work independently and collaboratively in the community and as a team member
- Great communicators dedicated to providing the best service for every person
- A broad understanding of mental health systems and legislation
- A Disability Positive Notice card (yellow card)
- A Children Check Positive Notice card (blue card)
- A current first aid certificate
- A current CPR certificate
- A current driver’s license
- Full Comprehensive Car Insurance

Pay: $43.00 per hour

Expected hours: 38 per week

Schedule:

- 8 hour shift
