We are seeking a skilled and compassionate Diabetes Educator to join our dynamic team on a contract basis **one day a week (Flexible)**. As a Diabetes Educator at East Keilor Clinic, you will have the opportunity to make a meaningful impact on the lives of our patients by providing exceptional care and services.

**Available Position: Diabetes Educator (Contractor)**

This position offers flexible working between Monday-Saturday as only one shift per week as required. Our practice operates Monday to Saturday, you will work with community and their families to educate and manage clients at risk of diabetes or living with diabetes. This includes care planning, multidisciplinary care coordination, service navigation and working closely with clients to get the best outcomes. You will be able to integrate culturally appropriate diabetes self-management education within the clinical care you provide to promote physical, social, spiritual and psychological wellbeing. You will also be able to confidently run health promotion activities as well as educational sessions with community to raise awareness about diabetes, healthy lifestyle choices and the services First Peoples' Health and Wellbeing offers. Experience using Best Practice is desirable but not required.

**Requirements**:

- Current AHPRA Registration
- Enthusiastic and eager to learn and develop
- Comfortable networking with potential referrers
- Proven track record in building and maintaining a clinical caseload

**Contractor Model of Employment**:
Take control of your employment and future with a contractor role. Our model offers you the ability to take home a percentage from each patient you see.

**Skills and Experience**:

- AHPRA registration
- Provider number
- Professional Indemnity Insurance
- Excellent interpersonal and communication skills
- Ability to work independently and as part of a multidisciplinary team
- Current ABN, Professional Indemnity and Liability Insurances
- Australian working rights
