**What is the role?**

As a Client Facilitator for the Gold Coast and surrounding area, you will ensure delivery of client focused services that effectively meet the customer’s needs, while promoting functional independence, wellness and lifestyle choices.

You will be required to:

- Undertake SCCQ Home Care intake and on-boarding process.
- Facilitate customer and network engagement in the customer service plan.
- Monitor and keep up to date all customer documentation.
- Coordinate to ensure that customer’s physical, social, emotional and clinical needs are met; including liaison with medical professionals as required.
- Analyse the engagement of each customer with SCCQ and suppliers.
- Implement strategies engaging customers to ensure compliance with clinical, support and professional
standards.
- Participate in the “On Call” system.
- Manage the implementation of support systems that encourage the involvement of relatives, friends and community to the extent defined by the customer.
- Maintain professional relationship with suppliers and contractors and monitor contractor performance and report workplace safety incidents and hazards.
- Liaise with internal and external provider representatives to improve service quality and administrative effectiveness.
- Oversee and approve individual client monthly statement within approved timelines and delegations.
- Coordinate / Case Manage care services for customers within budget targets.
- Complete mandatory training and requirements for annual certification to maintain knowledge and skill base.
- Assist in the induction of staff into SCCQ.
- Ensure Workplace Health and Safety processes are followed in accordance with SCCQ policies, procedures and processes.
- Promote education and workplace performance that support safe workplace practices.
- Other tasks as required.

**What you will bring to the role?**
- Qualifications in Nursing or Allied Health (desired)
- Current Senior First Aid and CPR (desired)
- Extensive experience with complex care or support management
- Strong knowledge of DSS Home Care Consumer Directed Care
- Strong conflict resolution and negotiation skills
- Ability to manage time effectively and prioritisation skills
- Excellent problem-solving skills with a solutions focus
- Previous experience working within Sales targets, upselling and cross selling
- Exceptional interpersonal skills and the ability to relate to a variety of people
- Good phone manner including the ability to develop rapport with clients
- Demonstrated ability to work autonomously
- Intermediate skills in Microsoft Office
