Provide support to strengthen a participant’s ability to design and build their personal supports with an emphasis on linking a broader system of support across a complex services delivery environment.
- Coordination of supports focusing on supporting participants to direct their lives, not just their services e.g. crisis and resilience development
- Assist participants to connect with NDIS supports, negotiate solutions with multiple stakeholders and build their capacity and resilience.
- Assist participants to develop knowledge, experience and connections with their community and broader systems of support.
- Work with the participant to assist them to understand plan funding and its purpose.
- Identify and understand participant confidence and skills assist participant decide what they want from services.
- Develop and design support solutions to meet participant outcomes.
- Assist the participant to identify and consider support options and link the participant to the broader systems of supports.
- Where practical, create a support and action plan to facilitate participant plan implementation.
- Assist to resolve points of crisis and develop capacity and resilience within the participant's network.
- Coach the participant through challenges.
- Assist participant prepare for the review and reporting of their achieved outcomes.
- Manage rosters, administration and intake process

**Job Types**: Casual, Full-time

**Salary**: $44,148.63 - $95,968.53 per year

Schedule:

- 8 hour shift
- Monday to Friday
- Public holidays
- Weekend availability

Supplemental pay types:

- Overtime pay
- Penalty rates

Ability to commute/relocate:

- Munno Para, SA 5115: Reliably commute or willing to relocate with an employer-provided relocation package (required)
