**About us**:
YAMHS is a standalone clinical in reach speciality Aboriginal mental health service with a multidisciplinary team that provides transitioning of care coordination, clinical yarning, cultural consultation and healing across the lifespan for HNE specialist mental health services within the Hunter New England Local Health District. YAMHS is delivered by mob for mob in partnership with Aboriginal communities across our footprint. YAMHS also provides community-based supports and consultations to people not referred though the in-reach pathway as indicated in the Aboriginal Mental Health & Social Emotional Wellbeing Operational Plan & Model of Care for best practice for Aboriginal people. YAMHS are seeking clinicians who are passionate about improving the mental health and social and emotional wellbeing of Aboriginal and Torres Strait Islander people accessing mental health services. This position works in the Cultural Healing Assessment Response Team within YAMHS and services the Calvary ED & Mater mental health centre and involves working closely with a dedicated team of professionals in a shared employment model. You will be supported and collaborate with staff from calvary ED, Mater MH services, Awabakal AMS, key stakeholders and YAMHS/CHART/Wiyiliin Ta team, ensuring a holistic and culturally sensitive approach to care that is provided in a wraparound support response.

**About the role**:
Your role will be pivotal in fostering strong interdisciplinary relationships and enhancing the quality-of-service delivery for Aboriginal and Torres Strait Islander patients/consumers admitted to Calvary ED and mater mental health centre by providing evidence-based mental health assessment, diagnosis and treatment to improve outcomes of patients and/or consumers and their families/carers. Whilst assisting services to deliver mental health care that has a holistic view of health, is culturally competent and responsive to the needs of Aboriginal and Torres Strait Islander people to support mainstream services to reduce readmission rates, did not waits and provide a clinical yarning approach that is holistic with care and helps coordinate a wraparound support response for our mob. Most of all you won’t be alone and will be part of a Deadly team who has the same commitment to improving outcomes for our mob, to increase early help seeking, reduce prevalence of crises and improve pathways and supports across the lifespan. For Aboriginal people, health is holistic, which links mental and physical health with a whole of life view, interconnected to spiritual and cultural factors, connection to land, community, traditions and identity which all helps keeps mob well.
