- Out Of Home Care Health Pathways Coordinator position.

Located on the lands of the Dharawal and Yuin nations, the Coordinator oversees the running of Out of Home Care Health Pathways team, ensuring that health assessments and plans are development for children and young people who are in Out of Home Care


We are looking for someone who has:

- Advanced knowledge and understanding of the health system in response to child in out of home care, and excellent knowledge of associated health pathways.
- Demonstrated experience in managing and implementing programs with multiple stakeholders, including Health, other government and non government organisations and Aboriginal Health Organisations
- Knowledge and understanding of child protection policy and practice in relation to the needs of children and young people in out of home care


This is a wonderful opportunity for a Health Manager will lead a small team, of clinicians providing assessments, developing health plans support for children and young people in Out of Home Care who have experienced violence, abuse and neglect,

From Helensburgh in the north to North Durras in the south, ISLHD covers an area of 5,687 square kms. That’s 5,687 square kms of beautiful beaches, coastal cycling tracks, lush rainforests, incredible bushwalks and friendly people. ISLHD operate eight hospital sites, employ over 8,500 staff and provide community health services across 58 locations. Commute times are short, childcare and schooling options are plentiful, and Sydney is close by.

The role is based at Urunga House in Wollongong, with some travel to other Illawarra Community Health Sites across both the Illawarra and Shoalhaven. A current, unrestricted NSW driver’s licence with the ability to travel to multiple locations is required.

The current funding is until June 2025, however funding has been recurrent for over 10 years and is expected to continue

**Selection Criteria**
- Describe your leadership style, experience, advanced knowledge and understanding of the health system in response to child in out of home care, and excellent knowledge of associated health pathways.
- Outline your experience and knowledge of project management, design and implementation of new models of care, service delivery, service planning
- Outline your negotiation and networking skills, particularly maintaining positive relationships and partnerships both within ISLHD as well as working within a cross-agency environment, including working alongside Aboriginal organisations.
