About The Role:

- Establish & maintain relationships with clients and families
- Ensure Quality & Compliance through conducting regular visits
- Meet with clients and families to establish their needs and identify their options
- Create and evaluate care plans
- Work closely with the Rostering team to ensure the correct skill mix for clients to ensure their needs are met
- Build and maintain relationships with stakeholders (internal & external) to be able to provide a range of services

What we need from you:

- The ability to build strong relationships and networks within the community - Must have a strong understanding of the Aged Care Sector as well as Home Care Package guidelines and service requirements
- Excellent customer service
- A current valid driver's licence, registration and reliable vehicle
- Strong service coordination and organisation skills
- A clinical background (e.g., EN, RN or Allied Health) (Preferable)
- Must be able speak Mandarin or Cantonese

**Salary**: $55,000.00 - $75,000.00 per year

Supplemental pay types:

**Experience**:

- Case management: 1 year (preferred)

**Language**:

- Mandarin or Cantonese (preferred)
