**The Role**:
Due to growth we are looking for a Home Care Service Advisor reporting to the Home Care Regional Manager to support the coordination and management of care to our clients who are on Home Care Packages in the Nepean and Mountains areas.

As a Home Care Service Advisor you will:

- Complete holistic assessments
- Develop appropriate care plans
- Manage client’s care needs within Home Care Package funding
- Manage referrals to other services when required
- Manage and provide service delivery to a portfolio of home care clients and their families.
- Cover leave for other Registered Nurses and Service Advisors clinical care and assessments

**About You**

To be successful in this role, you will have:

- Qualified Enrolled Nurse or Registered Nurse
- Excellent communication and interpersonal skills
- High level of organisational and time management skills
- Intermediate-Advance Microsoft Office skills
- Experience in home care, with a qualification in Community Services, Aged Care, Community or Case Management (highly desirable)
- Driver's license and car with comprehensive insurance (required)
- Evidence of COVID-19 vaccinations
- Evidence of flu vaccinations
