Documen Support Services is a Disability Service Provider based in the Liverpool region. We are currently expanding our team and seeking experienced support workers with the skills to hit the ground running. We are currently expanding our individual support staff and social based day programs.

**POSITION OVERVIEW Support Worker**

The Disability Support Worker is responsible for implementing training and support services aimed at maximizing the living, employment, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.
- Support clients to participate in age-appropriate activities and tasks including employment, in home support and community access.

**RESPONSIBILTIES AND DUTIES**
- Support clients to access activities and outings in the community including but not limited to swimming, bowling, horse riding and other activities as deemed necessary and appropriate by clients and/or their representative(s). This is achieved through individual or group supports.
- Provide direct assistance to each client in accordance with their service plan and as directed by Documen senior management.
- Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required. Incident reporting and medication management documentation for individuals that require monitoring.
- Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
- Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.

These roles are sub-contractor based and require the following:

- In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals (BSP).
- Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
- Assume the role of key worker for designated clients, including the preparation of a quarterly report on client progress.
- Liaise extensively with other staff to ensure the overall smooth and consistent running of the service (WhatsApp).
- Report regularly to manager (Daily - WhatsApp).
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review (online meetings and social media included).
- Provide back up and support to other staff as necessary (Flexibility).
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Notify the service if you are unable to work your rostered shifts, with as much notice as possible.

These roles are sub-contractor based and require the following:

- Worker Screening Check
- Police Clearance
- ABN
- Drivers Licence (Valid)
- Reliable Vehicle (For client transportation)
- NDIS Workers Orientation Module
- Any other relevant documents as required by Documen Management

**Job Types**: Subcontract, Casual

**Salary**: $33.00 per hour
