Care Finder Coordinator
We are seeking a Care Finder Coordinator to join our robust Community Support Services Team. The position will work in collaboration with other aged care, health and support services agencies to provide specialist and intensive assistance to clients.
Hours:
- Permanent Part time Position 50 hours per Fortnight
- Social Community Home Care and Disability Services Award Level 5
- Base hourly rate from $44.92/hour, plus 10.5% superannuation and salary packaging
Key Accountabilities include:
- Provide assertive outreach service support in collaboration with other aged care and support service agencies.
- Facilitate appropriate transition from the Assistance with Care and Housing program to the Care Finder program.
- Collect data and information consistent with care finder policy, guidelines and data managements and assist clients with My Aged Care system
- Prepare regular Capital Health Network Reports and participate in evaluation activities as directed by CHN
- Attend CHN arranged meetings and maintain strong working relationships with relevant CHN staff.
- Provide high-level client case coordination support through periodic check-in communication and follow-up support once services have commenced.
- Participate in quality assurance and continuous improvement processes for the Community Support Services team.
Care Finder Coordinator Requirements
- Diploma qualifications in a relevant field such as Community Services or Social Housing or equivalent work experience.
- A.C.T. Working with Vulnerable People Card
- Current Driver Licence and own car
- Current First Aid Certificate
To Be success in this role, you will have:
- Demonstrated thorough understanding of relevant national professional standards and guidelines.
- Demonstrated sound knowledge of the local community services sector and aged care resources.
- Demonstrated sound understanding of risk assessment and duty of care for client services.
- Demonstrated diverse communication skills including the ability to liaise effectively with other agencies, and demonstrated ability to relate to individuals in need of support.
- Extensive experience working with diverse and vulnerable population groups including but not limited to LGBTI, Culturally and Linguistically Diverse (CALD) and Aboriginal Torres Strait Islander (ATSI).
- Demonstrated excellent administration skills and computer literacy.
