**About the Role**:
An exciting opportunity exists for a suitably qualified Allied Health professional to provide care coordination and leadership for patients receiving care under the general medicine service with complex care needs.

This role will work closely with the allied health team, unit CNC and medical team to problem solve discharge barriers, coordinate patient care with the treating team and advocate to external agencies to facilitate safe and timely discharge.

This role will coordinate the Allied Health team on the unit and participate in activities to represent Allied Health on the unit. This role involves liaising across Canberra Health Services, partaking in service improvement and quality assurance initiatives and contributing to the supervision and training of fellow Health Professionals and Students. The Complex Care Coordinator will have highly developed professional knowledge of complex discharge planning, service options and availability across the ACT and region, with the ability to lead multidisciplinary teams and provide education and supervision to a broad range of staff.

- Undergo a pre-employment National Police Check.
- Current registration with AHPRA for AHPRA registered professions.
- Current registration issued under the _Working with Vulnerable People (Background Checking) Act 2011_ is required for any unregistered allied health professionals
- Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
- Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.
