**About the opportunity**

Reporting directly to the Chief Operating Officer in the executive team, the role of the Disability Manager - CC Homes is to provide stewardship over disability services to ensure that current CareChoice clients receive supports in line with the goals, aspirations and funding. This also includes ensuring the timely review of client support plans and support coordination plans, timely submission of progress reports and oversight of disability support programs to ensure client satisfaction and retention are of the highest standard.

The Disability Manager - CC Homes is responsible for overseeing disability programs from 6 weeks of service delivery onwards. For clients with complex needs, this may include intensive oversight liaising with a number of stakeholders to ensure the smooth ongoing running of programs.

The Disability Manager - CC Homes will work closely with the Chief Operating Officer, Client Engagement Managers, Disability Manager Complex, key business stakeholders and Team Leaders managing Client Programs/ Houses to ensure delivery of supports is seamless from initial intake through to long term program running.

As the Disability Manager - CC Homes with CareChoice, you will complete with some of the following tasks (not limited to):

- Ensuring adherence to performance targets (financial & non financial) budgets and operating in line with budget expenses
- Ensuring that all clients have current support plans and that these support plans are in line with the client’s goals.
- Ensuing the services delivered are in line with CareChoice’s Model of Support, & NDIS Practice Standards and NDIS Code of Conduct progress reports are completed and tracked by Support Workers.
- Providing day-to-day management of Team Leaders including regular 1:1 meetings/ supervision, learning and development support, and performance management.
- Ensuring that opportunities are maximised for each permanent client; this involves monitoring utilisation and adherence of funding and notifying stakeholders where there is a change in circumstance requiring an NDIS / TAC / WorkSafe plan review during the ongoing service provision.
- Key focus on customer satisfaction levels, appropriate oversight of disability programs, a focus on continual improvement and adherence to complaints & feedback policy
- implementing any recommendations agreed to.
- Building capacity and capability within the direct team and support worker base to ensure service delivered is in line with CareChoice’s Model of Support, NDIS
