The Opportunity_**
At Infin8 care, our Care Managers are a critical member of the centre leadership team. They work closely with the Care Coordinator and lead a team of dedicated professionals who aim to deliver the highest standards resident-centric care and clinical services in a timely and compassionate manner. This then enables our residents to make choices and have control over their support needs. At an operational level, the role involves
- Welcoming new and prospective residents and their families to the facility
- Generating care plans and conducting clinical assessments
- Developing AN-AAC submissions
- Working in collaboration with the Facility Manager to ensure that any complaints or issues are managed in a timely and resolution focused manner
**_ About You_**
You may be a Care Coordinator looking to take the next step, or experienced Care Manager who is wanting to build a long term career with an organisation that values its people. Either way, you are an excellent communicator, who is aligned with our philosophy and values. You can "think on your feet", and lead, guide and mentor others to ensure the very best outcomes. You build rapport with residents easily and take pride in their happiness. In addition to this you
- Have a valid AHPRA Registration
- Have experience in preparing and conducting clinical assessments and developing care plans
- Are experienced in and familiar with AN-AAC and associated compliance requirements
- Are willing to work to meet the needs of the residents and facility
- Hold valid NDIS Workers Screening Clearance (or have the ability to get one)
