**The Role**:
This position is for long service leave coverage on a maximum term contract.

**Role Purpose**:
Under the supervision of the Lifestyle Coordinator and the Residential Care and Service Manager/ Facility Manager and in conjunction with care staff, consumers, relatives and volunteers and other agencies, assist in the development, implementation, delivery and evaluation of a suitable and individually assessed social and lifestyle support activity program for consumers of the facility that promotes the whole person’s well-being with a focus on the consumers’ needs, goals and preferences.

**Key Duties and Responsibilities**:

- Ensure interactions with consumers are kind, caring and respectful of each consumer’s identity and culture.
- Treat consumers with dignity and empathy.
- Assist consumers make day-to-day choices that enable them to live the life they choose so they have control over their lives, including the activities they participate in.
- Support consumers diversity and what it means for them
- Deliver lifestyle programs as planned, involving, engaging and encouraging consumers to participate.
- Document consumer outcomes clearly and as per the Baptcare procedures.
- Able to recognize changes in a consumer’s health status and report those appropriately.
- Adhere to all the relevant workplace safety laws and regulations.
- Participate in the development of the lifestyle program, to ensure programs reflect consumer’s interests and culture.
- Work cooperatively with colleagues and adheres to Baptcare HR policies in relation to employee relations.
- Effective and respectful communication.

**Essential Qualifications & Requirements**:
A Certificate IV in Leisure and Health or a similar qualification is highly desirable.

Have a minimum of 12 months of experience working in an Aged Care facility delivering social and recreational activities and programs to seniors and a background in providing support to residents with cognitive impairment.

Demonstrate a customer-centric approach with a genuine passion for working in Aged Care.

Have a genuine passion to work with the elderly, fabulous communication and team work skills and basic computer skills.

Display exceptional attention to detail, integrity, reliability, and maintain a positive and adaptable attitude.

Exhibit a track record of maintaining confidentiality and working effectively within a team.
