**About the facility**
- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.

**About the role**
- An exciting short term opportunity to join the interprofessional Connected Care team, providing care for clients with chronic and complex conditions in the community. The team provide comprehensive assessments, care coordination, linkages to services and some specialist allied health and medical care. The model of care is responsive and agile to client and organisational needs. The team includes care-coordinators, nurses, medical staff and allied health professionals including psychology, diabetes educator, physiotherapist, and social workers. This role is an outreach role, with most consumer care being provided in their home in person, or via phone, or telehealth.
