**Role Description**

We are looking for a passionate individual to join our team in a part time capacity with the opportunity to work between the Bendigo & Castlemaine offices.

You must be eager to learn and excited by the opportunity to make a difference in the lives of older Australians and individuals who have complex care needs, living in our regional communities.

The primary purpose of this role is to provide effective and high quality care coordination to Home Care Package and NDIS consumers that is safe, effective and promotes independence.

You will enjoy a varied and diverse role working collaboratively with our consumers in the delivery of aged care and disability packages to individuals who have complex care needs and who are at risk of premature or inappropriate residential care or whose quality of life is adversely affected by their disability.

This role includes developing effective and supportive relationships with both internal and external stakeholders to plan, promote and deliver responsive and flexible individual supports, with a focus on enhancing and supporting independence in everyday life to enable them to continue to live in their own home environment.

You will use local knowledge of other community services, enabling you to effectively collaborate with clients to understand their care goals, develop care plans to meet their needs and liaise with service providers to adjust services as needed.

**The Team**

The Community and Public Health Services Division has a primary focus on improving the health outcomes of the communities in our region. With a range of local and regional programs supporting place-based health promotion, prevention, care and support, they work collaboratively with other Divisions and regional partnerships and networks to support quality outcomes. The Division has a strong leadership role in the Loddon Mallee Region and is committed to ensuring processes are in place to deliver on our Division.
