**The Role**:
As a Case Manager (Clinical), you'll be at the forefront of creating a happy, safe, and welcoming environment for those in our Melbourne-based retirement communities.

In this critical role, you'll deliver exceptional person-centred care and support, working with a team of experienced professionals. You'll assess and evaluate the care and services provided, ensuring each resident's needs and preferences are met. With your excellent communication and leadership skills, you'll build strong relationships with residents and external providers to make sure everyone feels at home.

**Requirements/Skills**
- Current AHPRA registration
- Full Australian Working Rights
- Management experience in Aged Care or Home Care - desired
- Sound knowledge of Home Care packages
- Registered Nursing qualification
