Home Support Assessor

As a Home Support Assessor, supporting clients in Werribee and surrounding areas, you will conduct face-to-face home support assessments with older Australians to enable access to the care they need to remain living independently in their chosen community. You will use a standardised national assessment process that focuses on wellness.
Working closely with the client, you will develop support plans with individualised goals. You will match and refer older adults to services which can assist them to achieve optimal wellness and healthy ageing.

- Competent computer skills - you’ll be inputting information into a laptop
- Ability to work autonomously - Assessments are done in person, at client’s own home
