**Who are we?**

At Bupa Telehealth, we help our customers manage their chronic conditions through behaviour change techniques and provide more clinical support to customers after a hospital admission. These telephone-based programs are delivered by our Nurses and Dietitian health coaches, who will use their clinical skills and knowledge to help keep our customers out of hospital. Our programs are designed to help our customers feel supported, informed and understood as an active participant in decisions around their health and wellbeing.

Bupa Telehealth Team Leaders lead our nurses and dietitians in the delivery of the programs.
