Exercise Physiologist

**Position Title: Exercise Physiologist - Chronic and Complex Care Service**
**Employment Type: Maximum Term Part Time - 3 Year Contract**
**Hours per week: 24 hours per week**
**Position Grade: Exercise Physiologist Level 3 Year 1-2**
**Salary: $105,093 - 108,612 (pro rata) per annum**
**Enterprise Agreement: The Named NSW (Non-Declared) Affiliated Health Organisations’ Professional and Associated Staff Agreement 2009**

**Position Summary**:
A vacancy exists in the above unit for a highly motivated Exercise Physiologist -Level 3 with appropriate knowledge and skills to join our friendly and supportive team. Our progressive and innovative team offers development opportunities for our staff and a caring environment for the delivery of high quality patient centred care.

The successful applicant(s) will work collaboratively with other team members of the Community Palliative Care and Heart Lung Chronic Care Team, implement and provide community care services for people with late stage chronic degenerative conditions.

Specifically, the position will contribute to a holistic assessment of the consumer’s physical health, developing an individualised and tailored exercise program to meet the consumer’s physical health needs, and evaluate the effectiveness and acceptability of the program. The position will contribute to the development of resources for consumers, their families and carers, and clinicians.

The service operates Monday
- Friday. Day for working will be discussed at time of interview.

The vacancy is open to Australian citizens, New Zealand citizens who hold a valid New Zealand passport, Australian permanent residents and those non-residents deemed by the Department of Immigration and Citizenship to be eligible to work in Australia only.

**Application Instructions**:
**Selection Criteria**:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia
2. Registration as a Registered Exercise Physiologist with Australian Health Practitioner Regulation Agency (AHPRA), with at least 3 years relevant post registration experience Need to add ESSA registration
3. Demonstrate understanding of and commitment to issues relating to Palliative Care, Haematology/Oncology, Motor Neuron Diseases, COPD, Transplant or other life-limiting illness
4. Provide high quality assessment and develop tailored individual exercise programs for clients with Chronic Disease where appropriate within the context of primary health care in a professional, confidential and culturally safe manner
5. Demonstrate how you would provide follow-up monitoring and care as directed through health assessments and / or GPMP / TCAs Work as part of a multi-disciplinary team including GPs, Social workers, pharmacists and Cardiologists, Respiratory and Palliative Care Consultants and Allied Health staff.
7. Computer literate in Windows environment and health data systems
8. Current unrestricted Class C Driver's License.
