**About the Role**

As a Support Worker at Place of Hope Care, you will provide compassionate, personalised support to a **young adult male with autism**, assisting them in their transition to independent supported living. This **part-time** role involves delivering **3 hours of daily support, 4 to 7 days per week**, focusing on **establishing morning routines, encouraging healthy lifestyle habits, and fostering greater independence.**

**Key Responsibilities**
- Support the participant’s transition to independent supported living by building consistent routines and daily structure
- Assist with morning routines, including personal hygiene, getting ready for the day, and feeding pets
- Prepare or assist with healthy meals, encourage nutritional choices, and support positive eating habits
- Provide guidance with time management, organisation, and completion of daily tasks
- Support household cleanliness, including assistance with tasks the participant may avoid due to sensory sensitivities (e.g., unable to clean up after the dog)
- Foster independence while ensuring safety and emotional wellbeing
- Maintain accurate daily progress notes and communicate effectively with the care team

**What We’re Looking For**
- Certificate III or IV in Individual Support, Disability, or similar (preferred)
- Driver’s Licence
- Police Check (National)
- Car Registration and Insurance
- CPR Training Certificate
- First Aid Certificate
- Manual Handling Certificate
- Statutory Declaration
- Driver’s Declaration
- NDIS Worker Orientation Module Certificate
- Proof of Working Rights (WWC)
- NDIS Screen Check
