**Description**:
**Right At Home - Melbourne Frankston** is seeking compassionate and dedicated Personal Care Workers (PCW) to join our team, providing high-quality in-home care to clients in the Melbourne, Frankston, and Peninsula areas. This role offers an opportunity to make a meaningful difference in clients' lives, helping them live independently and comfortably at home.

**Responsibilities**:Deliver personal care based on each client’s individualised care plan, as directed by the Registered Nurse (RN).

May include:

- Assist clients with activities of daily living, including personal hygiene, grooming, dressing, and toileting.
- Prepare meals and support clients with nutrition.
- Provide companionship and emotional support to ensure clients feel valued and comfortable.
- Drive clients to appointments and social activities and travel between clients’ homes.
- Assist with light household tasks, such as cleaning, laundry, and tidying.
- Monitor and report any changes in clients' health or wellbeing to the RN.
- Document the services and care provided to the client.

**Requirements**:

- Minimum 1 year of experience in community or in-home care assistance.
- Dementia care experience (preferred).
- Current First Aid and CPR certification.
- Certificate III in Aged Care.
- Valid Police Check and Working with Children Check (must).
- NDIS Screening Check (preferred).
- Driver’s license and own car with comprehensive car insurance (must).
- COVID-19 vaccination.
