**Role Overview**:
We are seeking a dedicated and experienced Care Manager to join our team. In this role, you will be at the forefront of ensuring our clients receive the highest level of personalised care.

You will oversee the care provided by our team, manage client relationships, and collaborate closely with our Team Leader to ensure the efficient running of our Care Team. This position requires strong customer service skills, leadership, and a passion for client care.

**Key Responsibilities**:

- Lead and manage a team of Care Coordinators, ensuring smooth workflow and high-quality care delivery.
- Collaborate with the other care managers and Team Leader to optimise the efficiency and effectiveness of the Care Team.
- Oversee client care plans, regularly reviewing and updating them to reflect clients’ changing needs.
- Conduct in-home detailed care assessments to monitor and enhance the care provided.
- Collaborate with clients, their families, and healthcare professionals to develop and maintain comprehensive care plans.
- Handle escalated client concerns and resolve issues with professionalism and empathy.
- Ensure compliance with all relevant aged care standards and regulations, particularly in the context of Home Care Packages (HCP).
- Contribute to staff training and development, and participate in continuous improvement initiatives.

**Skills and Experience Required**:

- Minimum of 2 years of experience in aged care or a related field, with demonstrated expertise in care management.
- Strong customer service, communication, leadership, and problem-solving skills.
- Proven ability to manage multiple tasks effectively, with a high degree of organization and attention to detail.
- A valid driver’s license.

Pay: From $70,000.00 per year

**Experience**:

- Care management: 2 years (preferred)

Licence/Certification:

- National Police Certificate (preferred)
